card of admission on rehabilitation, certified by the seal of the sending institution;
detailed excerpt of the child’s development history;
copy of preventive vaccination card;
results of blood and urine general analysis (not older than 10 days); results of coproovoscopy and enteroscopy;
certificate of contact absence with infectious patients with a note about the absence of infectious skin diseases and lice (not older than 3 days);
certificate from the school about the educational program and year of study, journal and textbooks (for pupils during the school period).

voucher for hospitalization certified with doctor`s seal;
outpatient card (form112) or detailed excerpt from it;

fluorography for accompanying person caring for the child.
